For and in consideration of the donations made, I / me, the donor, agrees and authorizes the use of the account of the Commission on Filipinos Overseas at the Development Bank of the Philippines as the legitimate depository bank to which the funds for the project(s) will be remitted and transferred. The fund shall not be deposited to the National Treasury and shall be disbursed upon completion of necessary documentary requirements by the beneficiary as provided under COA Circular No. 2003-05 dated 21 November 2003.
Moreover, refunds, reduction of amount or subsequent cancellation of the donation shall not be honored in any case once the transaction is made final. Any discrepancy on the transaction must be reported within one (1) month, otherwise, the Commission shall not be held liable.
